New Jersey is a "No-Fault" state. This means your own auto insurance company pays for your medical treatment if you are injured in a car accident, regardless of who caused it. This coverage is called Personal Injury Protection (PIP).
But as of January 1, 2026, there’s a crucial decision every NJ driver needs to make about their PIP: Who should pay first, your car insurance or your health insurance?
When you set up your auto policy, you can select who is the "Primary" insurer for auto-related medical bills.
PIP Primary: Your car insurance pays first. This is the traditional and most expensive option.
Health Primary: Your private health insurance (like Blue Cross, Aetna, etc.) pays first.
Why you should care: Selecting Health Primary can save you up to 15% on your auto insurance premium. For a multi-car family, this is a massive saving that can significantly offset the recent 2026 rate hikes.
This savings sounds great, but it requires an expert review. You cannot select Health Primary if you are on:
Medicare
Medicaid
NJ FamilyCare
